Short answer: the evidence is promising but small. A 2026 systematic review pooled five randomized trials of creatine monohydrate in mental disorders; four were in major depressive disorder and one in bipolar depression, with 126 people on creatine and 112 on placebo [1]. Two trials were rated low risk of bias and three had some concerns [1].
The most encouraging result came from one trial: creatine added to escitalopram outperformed an SSRI plus placebo at eight weeks (Cohen's d = 1.13) [1]. Creatine as augmentation to CBT also beat CBT plus placebo, and a pharmacotherapy augmentation trial in adults also appeared positive, but other single trials found no difference from placebo, including one in female adolescents and one in bipolar depression [1]. Each of these comparisons rests on one trial, so the pooled picture is not yet solid.
What's shown so far is that creatine is tolerated across these small trials at doses of 2 to 10 g/day for 4 to 8 weeks [1]. What isn't shown is that it reliably treats depression. Trials of creatine in healthy adults have measured cognition and mood only as secondary outcomes, and their results are mixed [2][3][4]. A reasonable reading is that creatine is suggested as a possible add-on worth testing in larger, well-designed trials, not a treatment to rely on.
This is research explanation, not medical advice. If you're considering creatine alongside depression treatment, talk with your prescriber or clinician about your own situation.
